Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It collects and shares cleaned trial data and ratings using PRECIS-2 allowing for multiple and diverse meta-epidemiological studies that compare treatment effects estimates across trials that employ different levels of pragmatism, as well as other design features.
Background
Pragmatic studies are increasingly acknowledged as providing evidence from the real world for clinical decision making. However, the usage of the term "pragmatic" is not consistent and its definition and evaluation requires clarification. The purpose of pragmatic trials is to inform policy and clinical practice decisions, rather than confirm an hypothesis that is based on a clinical or physiological basis. A pragmatic trial should strive to be as close to real-world clinical practice as possible, such as its participation of participants, setting and design as well as the implementation of the intervention, as well as the determination and analysis of outcomes and primary analyses. This is a major distinction from explanation trials (as described by Schwartz and Lellouch1) which are intended to provide a more thorough confirmation of a hypothesis.
Truly pragmatic trials should not conceal participants or the clinicians. This can lead to a bias in the estimates of the effect of treatment. Pragmatic trials should also seek to recruit patients from a wide range of health care settings so that their results are generalizable to the real world.

The development of a PRECIS-2 tool that provides an objective, standardized evaluation of pragmatic aspects is a first step.
Methods
In a practical trial, the aim is to inform clinical or policy decisions by demonstrating how an intervention would be incorporated into real-world routine care. This differs from explanation trials that test hypotheses regarding the causal-effect relationship in idealized situations. In this way, pragmatic trials could have a lower internal validity than studies that explain and be more prone to biases in their design, analysis, and conduct. Despite these limitations, pragmatic trials can contribute valuable information to decisions in the context of healthcare.

Results
While the definition of pragmatism may not require that all trials are 100 100% pragmatic, there are some advantages to including pragmatic components in clinical trials. These include:

Koppenaal et al10 created an adaptation of this assessment, dubbed the Pragmascope which was more user-friendly to use in systematic reviews. They found that pragmatic reviews scored higher across all domains, however they scored lower in the primary analysis domain.
The difference in the main analysis domain could be explained by the fact that the majority of pragmatic trials process their data in the intention to treat method while some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the areas of organisation, flexible delivery and following-up were combined.
It is crucial to keep in mind that a pragmatic study should not necessarily mean a low-quality study. In fact, there is an increasing number of clinical trials that use the term "pragmatic" either in their abstracts or titles (as defined by MEDLINE, but that is neither sensitive nor precise). The use of these terms in titles and abstracts may suggest a greater awareness of the importance of pragmatism however, it is not clear if this is reflected in the content of the articles.
Conclusions
In recent years, pragmatic trials have been becoming more popular in research as the importance of real-world evidence is increasingly recognized. They are randomized studies that compare real-world treatment options with clinical trials in development. They are conducted with populations of patients closer to those treated in regular care. This method is able to overcome the limitations of observational research, for example, the biases that come with the reliance on volunteers, and the lack of the coding differences in national registry.
Pragmatic trials offer other advantages, like the ability to use existing data sources and a greater likelihood of detecting meaningful differences from traditional trials. However, these tests could have some limitations that limit their validity and generalizability. Participation rates in some trials could be lower than anticipated due to the healthy-volunteering effect, financial incentives or competition from other research studies. Practical trials are often restricted by the necessity to recruit participants in a timely manner. Certain pragmatic trials lack controls to ensure that observed variations aren't due to biases during the trial.
The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatism. They assessed pragmatism using the PRECIS-2 tool, which consists of the eligibility criteria for domains, recruitment, flexibility in adherence to interventions and follow-up. They discovered that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or more) in at least one of these domains.
Trials that have a high pragmatism score tend to have more expansive eligibility criteria than traditional RCTs that have specific criteria that are not likely to be present in the clinical setting, and include populations from a wide variety of hospitals. According to the authors, may make pragmatic trials more useful and applicable in everyday clinical. However they do not guarantee that a trial is free of bias. Moreover, the pragmatism of the trial is not a predetermined characteristic; a pragmatic trial that does not have all the characteristics of a explanatory trial can yield reliable and relevant results.
